The files that are needed to display the toolbar icons are not being found.
Solution
Please run adadmin
Maintain Applications Files menu
Copy files to destinations
Bounce mid-tier services and retest the issue.

Oracle Apps Forms is not showing Menu Toolbar containing the icons.
